Enrollment into one of Imperial Health Plan of California or Imperial Insurance Companies, Inc. (HMO) (HMO SNP) MAPD plans is easy. Please use one of the enrollment methods below:

Self Enroll with Imperial

Enroll directly through Imperial's own online application at members.imperialhealthplan.com/member/enroll. This same application also lets you shop and compare Imperial Medicare Advantage plans before you enroll.

Self Enroll with Medicare Online Enrollment Center (OEC)

Complete the online enrollment application via the CMS Medicare Online Enrollment Center (OEC) at medicare.gov. By using the OEC, you acknowledge that you've reviewed the Summary of Benefits and Drug Formulary and the Provider Search tool available on this site, or by calling an Imperial Health Plan representative at 1-800-838-8271.

Call us at 1-800-838-5914 (TTY: 711) and we'll complete the application with you over the phone.

Hours: October 1 – March 31, Monday – Sunday, 8:00 AM – 8:00 PM PST. April 1 – September 30, Monday – Friday, 8:00 AM – 8:00 PM PST.

Schedule a face-to-face appointment with an employed Sales Representative or Independent Agent by calling 1-800-838-5914 (TTY: 711).

Hours: October 1 – March 31, Monday – Sunday, 8:00 AM – 8:00 PM PST. April 1 – September 30, Monday – Friday, 8:00 AM – 8:00 PM PST.
