A doctor-founded health plan
built around the people it serves.

From a single founding question: what would a health plan look like if it were built by a clinician for the people they actually care for? Imperial has grown into a multi-state plan serving Medicare and Individual & Family members across the Southwest and California.

Our story

Imperial Health Holdings Medical Group was founded in 2013 by a physician who wanted to be part of the change he wanted to see in healthcare, an internal medicine physician and cardiac electrophysiologist who had spent a career on the clinical side, seeing again and again how the design of a health plan shapes whether members actually get well.

The idea behind Imperial was simple but uncommon: build a plan that pays for the right things, coordinates care like a clinical team would, and answers the phone when members call. Not a marketing pitch. An operating principle.

From that starting point, Imperial has grown into Imperial Health Plan of California (Medicare Advantage across 51 of California's 58 counties), Imperial Insurance Companies (ACA Marketplace in Arizona, Nevada, and Texas), and Imperial Health Plan of the Southwest (ACA Marketplace in Utah). Small Group plans launch in fall 2026 in Maricopa County, Arizona and Tarrant County, Texas.

The footprint has gotten bigger. The principle hasn't changed.
