Effective January 1, 2027, please use the following mailing address when sending Late Enrollment Penalty (LEP) premium payments only:

If you are enrolled in an automatic payment (auto-pay or bill-pay), please also review and update the billing address associated with your auto-pay/payment account to reflect the new mailing address above.

Updating your information will help ensure your LEP premium payments and related billing information are properly received and processed.
