We offer a Complex Case Management program for members who could benefit from more intensive coordination of services. Case managers are available to support members living with complex health conditions who need services and support to manage their health.
Case Managers partner with members and their providers to help manage their conditions with the goal of influencing illness trajectory, promoting adherence to the prescribed plan of care, and improving health outcomes. A personalized care plan is created based on the member's health goals, and case managers provide ongoing management and support as the member works toward those goals.
Common interventions include:
- Educating the member on their disease trajectory
- Reviewing risk and benefits of treatment decisions
- Helping to support and clarify goals of care
- Completion of a comprehensive medication review
- Fostering healthy patient/provider relationships
- Identifying and resolving barriers to care
- Assisting with community resources to address social determinants of health that may impede desired health outcomes
How the program benefits members:
- Provides one-on-one interventions with Imperial Health case managers and social workers
- Allows access to a multidisciplinary team — including registered nurse case managers, behavioral health case managers, social workers, and nonclinical care specialists — to help manage conditions and support important health services
- Helps coordinate health care services and communications between provider, family, and other care team members

Practitioner Credentialing & Rights
Practitioners are notified of their right to review and correct erroneous information obtained in the credentialing or re-credentialing process. This includes information from any outside primary source such as state licensing boards and malpractice insurance carriers. The right to review does not extend to references, recommendations, or other information that is peer review protected or where disclosure is prohibited by law.
Practitioners may ascertain the status of their application or reapplication at any time by contacting the Credentials Department at: credentialingadmin@imperialhealthholdings.com
Practitioners receive notification of their rights during the verification process or the appeal process if they do not meet criteria after receiving a denial or termination during the credentialing or re-credentialing process.
If credentialing information obtained from other sources varies from that provided by the practitioner, the credential coordinator will notify the practitioner in writing for their response within ten working days. The Credentialing Coordinator will make three attempts to collect the corrected information. Telephone, fax, email, or letter are all acceptable forms of communication.
Corrected information is accepted by the Credentialing Coordinator and documented in the credentialing system. The practitioner's application is pended until a decision is made by the Credentialing Committee.
If the Credentialing Coordinator is unable to obtain the requested information, terminated practitioners can correct discrepant information under the IMAS appeal policy. Practitioners are notified that appeals must be submitted within 30 days.
